We kindly ask that you e-mail or text a photo of the front and back of your insurance card, along with the name and date of birth of the subscriber before your visit . 

If we are not in-network with your plan and you would like to be seen at our office, upon receipt of full payment for the procedure, we will submit for you to be reimbursed.
